STANDARD VISIT

Current patients only

An appointment which includes medication management and psychotherapy. This is time to discuss progress, improvements, and concerns related to your medication and for us to make appropriate
adjustments. 30 min

EXTENDED PSYCHOTHERAPY

Current patients only

A longer appointment to address your concerns. Includes medication management where appropriate.

MEDICATION REFILL REQUEST OUTSIDE OF  APPOINTMENT

Please allow 48 hours for prescriptions outside of appointment times.

Note - controlled substance prescriptions are ONLY available during a scheduled appointment.

INSURANCE: I am able to accept Aetna insurance. Patients with out-of-network coverage may be eligible for partial or full reimbursement from their insurance companies. Payment in full is expected at time of service. You will be issued a “superbill” which you can submit to your insurer to seek reimbursement.

 

MISSED APPOINTMENTS: You will be charged full session rate for any appointment that is cancelled with less than 24 hours notice. You are required to have a credit card on file so that it can be charged automatically in accordance with the policy.

Prescription Requests: Please use the form found above to request prescriptions outside of session. Please allow 24-48 hours for scripts without an appointment. No controlled substance medication prescriptions will be made outside of appointment times.
